What to put on a babysitter information sheet
A good sheet answers the questions a sitter would otherwise have to text you about — or worse, guess at. Keep it to one page so it can live on the fridge.
1. How to reach you (and who's next)
List both parents' cell numbers, then one or two backups who live nearby and can actually show up — a grandparent, neighbor, or close friend. Add a short note on when to call each person, so your sitter doesn't hesitate.
2. Your address
It feels obvious, but in an emergency a 911 dispatcher will ask for it, and a nervous sitter may not know it by heart. Put it near the top.
3. Allergies and medications — for each child
Be specific: what the allergy is, what a reaction looks like, and exactly where the EpiPen or inhaler is. If a medication is due while you're out, write the dose and time.
4. Bedtime and routines
Kids test new sitters. "Bedtime is 8:00, two books, nightlight on, door cracked" ends the negotiation before it starts. Include anything that helps a child settle — a favorite stuffed animal, a song, a sound machine.
5. Food and screen rules
What's for dinner, what snacks are OK, and how much screen time is allowed. Clear rules make your sitter's job easier and keep things consistent for your kids.
6. Emergency basics
Poison control (1-800-222-1222 in the US), the nearest hospital or urgent care, and where the first-aid kit is. Our sheet pre-fills poison control for you.
Tips for a smooth night
- Walk through the sheet together when your sitter arrives — five minutes saves a dozen texts.
- Text them the phone link as well as leaving a printed copy, so the info goes wherever they go.
- Update it once a season. Bedtimes, allergies and favorite foods change faster than you'd think.
Questions
Is this babysitter sheet really free?
Yes. You can fill in, print and share the core sheet for free, with no account. The optional Full Kit adds extra sections, unlimited kids and pets, design themes, and removes our small footer.
Where is my information stored?
Only in your own browser. We never receive it. Phone links carry the info inside the link itself, so anyone you send the link to can read it.
Can I save it as a PDF?
Yes — tap “Print / save as PDF” and choose “Save as PDF” as the printer.
